Building Healthy Communities
This work is currently supported through numerous grant-funded initiatives housed within the Health Promotion Center, including Creating Healthy Schools and Communities and the Living Tobacco-Free initiative.
Through these programs, Health Promotion Center staff work collaboratively with school districts, businesses, communities, hospitals, and healthcare provider organizations to support healthy lifestyles for our patients and community members. Specific strategies focus on policy, systems, and environmental changes that reduce risks for chronic disease and improve quality of life for both children and adults.
Glens Falls Hospital has worked to create healthier communities since its founding in 1897. As part of this ideal, numerous community health planning efforts are coordinated by the Health Promotion Center to communicate community benefit programs to the public:
The Community Service Plan for 2013 - 2015 is required by the NYS Department of Health. A description of our progress on the plan can be found in the Community Service Plan Updates for 2014and Community Service Plan Updates 2015. The Community Health Needs Assessment (CHNA) and Implementation Strategy address the requirements set forth by the Internal Revenue Service through the Affordable Care Act, as well as the American College of Surgeons Commission on Cancer. An appendix to the CHNA is also available.
Glens Falls Hospital partners with AHI to conduct regional planning and community health planning activities. HealthyADK is a regional population health and socio-economic indicators resource portal maintained by AHI in collaboration with community organizations and agencies. This site provides easy access to up-to-date community health data for numerous sectors including community members, business, community development, government, educators, community-based organizations, and health care professionals.

Good Move is a campaign to encourage individuals and families to take steps toward good health in the community, in the workplace, and in the school. Being active, eating healthy foods and making use of preventative care may be easier than you think! ‘Fix It’ is a campaign of the Living Tobacco-Free initiative at Glens Falls Hospital. This campaign aims to encourage our community members to work together to fix the major health and economic implications of tobacco-use through education, mobilization and policy change. We collaborate with health care providers, local leaders, employers, landlords, youth and local organizations to help create tobacco-related policies that result in more smoke-free environments, a reduction in exposure to tobacco marketing, and healthier communities.
